If you have multiple Parents/Guardians that will be involved with the band or sending in payments please have each complete the form.
The Band Boosters will review your application and then you will receive a registration email. You MUST click that link to verify your account and complete the registration.
If multiple Parents/Guardians complete this form you can link your accounts as a family when you verify your registration.

First Name = Parent/Guardian First Name
Last Name = Parent/Guardian Last Name
Email = Parent/Guardian Email. You will be able to set how you want this used when you complete registration.
Phone = Parent/Guardian Phone. You will be able to set how you want this used when you complete registration.
MS Graduation Year = The year your youngest current Bishop Band student will graduate.
Student 1 = Full name of your band student
Student 2 = Full name of your 2nd band student, when applicable
Student 3 = Full name of your 3rd band student, when applicable
